You may prepare your answers in advance and submit them at once after you register or work on them over time, making sure you save your progress every time you update the form.

About the Nominator

This is where you enter information about the nominator.

  • Nominator’s name
  • Nominator’s contact email address
  • Nominator’s phone number
About the candidate

Enter information about the candidate. You will also require to upload an image of the candidate.

  • Candidate’s name
  • Candidate’s photo – The face and shoulders of the candidate should occupy most of the space in the image. The camera should be at eye level. Aim for an image size of at least 1MB.
  • Candidate’s email address
  • Candidate’s phone number 
  • Candidate’s address
  • Candidate’s company/organisation
  • Department/key function
  • Date entering the industry
  • Why do you wish to nominate this candidate?
  • Why should this candidate be considered for this award?
Testimonies

This section is where you will enter a two testimonies. For both testimonies you will need to enter the following:

  • Who the testimony is from
  • Their company name
  • Their email address
  • Their contact number
  • Independant testimony
